Utilization
Keeping care in the network speaks to the effectiveness of the
Health Connect plan design.
With the goal of cultivating strong, effective patient-provider relationships, Health Connect utilization
patterns demonstrate shared success in engaging patients in their health and ongoing care within EHN.
The Health Connect plan is designed to foster patient engagement and preventative care and enhance
communication among each patient’s personalized care team — helping to ensure collaboration is
streamlined across the full continuum of care for the best possible experience and outcomes.
Establishing care with a Primary Care Provider (PCP) is an effective tool for increasing patient engagement in
preventative care and navigating a complex health care system. Our Navigators begin this process, and when
necessary, the work is continued by our RN Care Managers and Social Workers. Health Connect Members
receive concierge care at every step. For 2023, EHN is working to better understand the demographics of
members who seek care out of network so that we can optimize efforts to engage them with in-network care.
