EHN Care Management
Helping a member recover from agoraphobia.
EHN Care Manager Jen Riley identified this member
through a Health Connect list of members with
Major Depressive Disorder (MDD) and/or anxiety.
The man was in his mid-30s and had a past history
of significant depression and anxiety. Jen contacted
his PCP and received referral for Care Management.
The patient continued weekly phone calls with Jen
during which they explored strategies to increase
his coping techniques for anxiety, depression, and
agoraphobia. Eager to return to bike riding, the
member began maintenance on his bike to make
it road worthy again.
During intake, Jen discovered the member had
depression and anxiety leading to crippling
agoraphobia, which led to work and marital strain
and poor self-image. His conditions developed from
childhood trauma and resultant strained family
relationships. The member reported that, during
the previous year, he had disengaged from his
therapist due to a poor fit and had stopped taking
prescribed medications.
Six weeks after beginning work with Jen, the
member left his house for the first time in months
so that he could go for a bike ride. The success of
this bike ride spurred him to make an appointment
with his Primary Care Provider (PCP) to discuss his
overall health. Within three months, the member
was riding his bike on a regular basis and
continuing to add distance, biking up to 15 miles.
The member reported his marriage was improving,
he was making small steps toward setting
boundaries and mending relationships with family.
At the time of the initial phone call, the member
told Jen he had not left his home, for any reason,
for nearly six months.
The member told Jen he was only operating at
10% of his full capacity. He also expressed a strong
desire and commitment to improve his mental
and physical health. Together, the member and
Jen crafted initial goals to seek out and develop a
strong relationship with a mental health provider
and resume bike riding, which the member had
previously found therapeutic.
Jen consulted with an EHN Social Worker to explore
resources for mental health providers. The patient
preferred to take an active role in interviewing and
connecting with providers to help himself find the
right fit. He was successful engaging with a provider
utilizing an app and set up weekly virtual therapy.

Four months after the patient enrolled with Care
Management, things had improved with work,
he had a strong relationship with his mental
health provider, was regularly riding his bike for
up to 20 miles at a time and described his health
as operating at 80% capacity. The member felt
confident that he was ready to graduate from
Care Management and expressed gratitude for
the assistance in accomplishing his goals.
Six weeks after beginning work with
Jen, the member left his house for the
first time in months so that he could
go for a bike ride.
