Patient care team
At EHN our care management framework puts our members at the center
of their care — forming a dedicated, comprehensive team to meet the
member’s health and wellness needs.
Our care team is designed to facilitate strong provider-patient relationships and drive collaboration among
providers, with technology to support communication and better outcomes. We believe in surrounding our
members with an expert, comprehensive team that truly cares. Premera supports the Health Connect plan
goals by providing comprehensive data to facilitate the EHN processes.
Navigator
• Dedicated EHN team working in partnership with Premera Customer Service to support Health
Connect Members in selecting Primary Care Providers, finding specialty care providers, basic benefit
questions and serves as liaison to members.
• Serves as the primary contact to coordinate care with providers and other members of the care team.

• Works directly with patients who need more robust support and additional resources to develop
self-management skills and improve their health trajectory.
• Along with PCP, develops the care plan, coaches patients and families, and assists in health
system navigation.
• Assesses psychosocial needs, assists with community care arrangements and reducing barriers
to care by connecting patients with organizational and community resources.
• Primarily providing telephonic services, coordination and planning.
• Works in partnership with leadership, quality specialists and clinic resources to meet quality
initiatives, and drives clinical strategy.
• Develops and implements workflows to improve patient outcomes.
• Serves as a knowledge base for quality-related care.
• Partners with the EHN RN Care Manager and clinic staff to coordinate care for the patient by
scheduling appointments to close gaps in care/clinical maintenance activities.
• Coordinates external documentation for patients who have received care outside of the network
or primary care setting.
“We’re interested in making ongoing adjustments to our processes to make it easier
for patients and providers to interact across the network. For example, in the
coming year we’re hoping to simplify how members interact with waivers when a
Health Connect Member needs care that we can’t provide in EHN.”
