ABNORMAL OPERATING CONDITIONS (AOCs)
SECTION 1:
GENERIC AOCs and REACTIONS
Examples of abnormal operating conditions that employees must recognize and react to include, but
are not limited to:
Pressure Related Conditions:
Recognize:
Uncontrolled leakage of natural gas: The uncontrolled release of gas that constitutes an
emergency, a burn through/penetration during welding or fusion, a corrosion leak or other
damage to the pipeline.
Unexplained pressure deviations from normal: The unintended increase, decrease, or
absence of pressures, particularly pressures exceeding the MAOP, or the unplanned shut-off
of service to customers.
Conditions requiring shutdown or pressure reduction in a pipeline: Non-Leaking defects
including corrosion or mechanical damage exceeding code required minimum pipe wall
thickness for a given pipeline stress level, air/gas mixture in the pipeline, or a hazardous gas
(other than natural gas) in the pipeline.
React:
•
Call 911 if situation warrants (i.e. risk to life or property),
•
Secure area, and
•
Contact immediate supervisor to have qualified individual(s) respond to
AOC location for remediation.
Material/Equipment Failure Conditions:
Recognize:
Material failure or defect: Failures that could result in a hazard(s) to persons, property, or the
environment such as severe atmospheric, external, or internal corrosion (< 30% of wall left),
stress corrosion cracking, discovery of non-code compliant material in the pipeline, exposed
pipeline, or significant loss of pipeline coating.
Malfunctioning component or component failure: Relief valve failure, monitoring regulator
failure, district regulator station failure, failure of odorization equipment, or other failures that
could result in a hazard(s) to persons, property, or the environment.
Loss of protection on the pipeline: The discovery of missing CP wires or equipment, CP
equipment shutdown or failure, stray current on the pipeline, or low CP levels which could
result in a hazard(s) to persons, the pipeline, other property, or the environment.
Under-odorization of the pipeline: Where odorant levels are at less than code-required
detection levels.
