Referral
A written order from your primary care
doctor for you to see a specialist or get
certain medical services. In many Health
Maintenance Organizations (HMOs), you
need to get a referral before you can get
medical care from anyone except your primary care doctor. If you don’t get a referral
first, the plan may not pay for the services.

A plan through Maryland Health
Connection that covers 70% of health
care costs, and you pay 30%. It’s also
the only plan level that may provide costsharing reductions.

Summary of Benefits and Coverage
Created as part of the Affordable Care
Act, it is an easy-to-read summary that
lets you see comparisons of costs and
coverage between health plans. You can
compare options based on price, benefits
and other features that may be important to you. The SBC is a snapshot of
a health plan’s costs, benefits, covered
health care services, and other features
that are important to consumers. SBCs
also explain health plans’ unique features
like cost sharing rules and include significant limits and exceptions to coverage in
easy-to- understand terms.
Specialist
A physician specialist focuses on a
specific area of medicine or a group of
patients to diagnose, manage, prevent
or treat certain types of symptoms and
conditions. A non-physician specialist is a
provider who has more training in a specific area of health care.
Tax Credit
A tax credit, or Advance Premium Tax
Credit (APTC), can reduce what you pay
for health insurance. When you apply
for coverage through Maryland Health
Connection, you’ll estimate your household income to find out if you qualify for
an advance payment of the premium
tax credit that lowers your premium (the
amount you pay each month to your
insurance plan).
Value Plans
Value plans are health plans that offer
lower deductibles and useful coverage
for more health care services before
your deductible is met. Value plans are
designed to lower your out-of-pocket
costs for the health care services the
majority of people use most frequently.
All value plans are required to cover core
benefits with a copay before you meet
your deductible and preventive services
for free. Plus, depending on the value
plan you choose, some services are available before paying your deductible. Some
require only a copay, even if you haven’t
met your deductible.
