ELIGIBILITY AND ENROLLLMENT
Important Note:
This Plan includes an automatic enrollment feature. All Eligible Employees that do not have
a deferral contribution rate of 6% or higher will automatically be enrolled into the plan with a
deferral contribution rate of 6% after 30 days of employment. Your deferral will be invested in an
appropriate Target Date fund based on your year of birth.
If you wish to make changes to the deferral percentage or do not wish to participate in the Plan,
you must opt out via the website www.CertifiedDifference.com by using the web login
instructions.
Who is eligible to participate in the plan?
PLAN HIGHLIGHTS
Any employee shall become a Participant in the Plan on the date on which he
first performs an Hour of Service for the Employer (the "Employment
Commencement Date").
When can I begin contributing to the Plan?
Eligible participants can begin contributing to the Plan on the first pay period
following employment.
Are rollovers accepted?
The Plan may accept a Rollover Contribution made on behalf of any
Employee, regardless of whether such employee is contributing to the plan.
How do I enroll in the Plan?
You can enroll via the website www.CertifiedDifference.com by using the web
login instructions found on page 7.
