ELIGIBILITY AND ENROLLLMENT
Who is eligible to participate in the plan?
All employees, with certain exclusions, are eligible to participate in the Plan.
Excluded employees are union employees, nonresident aliens, and those that
are employees as a result of a transaction described in Code section 410(b)(6)(C).
When will I be eligible to participate in the plan?
You are eligible to participate in the plan when you have reached age 21 or older
and have completed 1 year of service, during which you have completed at least
1,000 hour of service.
PLAN HIGHLIGHTS
When can I begin contributing to the Plan?
You may begin contributing to the Plan on the first day of the month
coincident with or next following the date when the eligibility requirements
have been met.
Are rollovers accepted?
The Plan may accept a rollover contribution for any eligible employee
regardless of whether the age and service requirements have been met
How do I enroll in the Plan?
You can enroll via the website www.CertifiedDifference.com by using the web
login instructions found on page 9 or by completing the forms located within
this book and returning them to your Human Resource representative.
