Prior to being required by the Indonesian Minister of Health Regulation No. 24 of 2022, electronic
medical records (EMR) had been implemented in a few healthcare facilities in Indonesia, both in
hospitals and primary care, as independent initiatives. The currently available information system does
not yet facilitate family physicians in providing holistic and comprehensive services.
The aim of this research is to develop an information system in primary care that will assist family
doctors in providing comprehensive holistic services. The first stage of this research was to develop
a model by observing and interviewing health workers at several primary care facilities to inventory
needs, then adding the function of assessing the risk of family stunting. The second stage is compiling
an information system, and the third stage is testing at primary care facilities.
An information system for primary care facilities has been formed, consisting of five major parts in the
form of integrated modules, namely the registration module, the initial examination module, the nurse
module, the physician module and the management module. The uniqueness of this application is
that in the registration section, if it is completely filled out, the risk of suffering from diabetes can be
identified, as can the families at risk of stunting based on the criteria of woman of childbearing age,
an inadequate household and no family member who earns money within six months. In the initial
examination module, reasons for encounters are coded based on the International Classification
of Primary Care 2. The diagnosis is documented based on clinical and psychosocial aspects, in
the physician’s module. The management module is equipped with items involving doctors with
family from the genogram model, in addition to pharmacotherapy. The information system has been
successfully tested at the primary care facilities.
