WE PARTNER WITH CLINICIANS TO
INFORM CARE DELIVERY
Through major advances in technology, health
systems reform, and changing demographics,
new challenges face global health systems and
governments. A pressing need to use more
detailed system-wide analytics, the creation
of longitudinal patient records, enhanced
patient safety and experience, and the need to
create interoperable information flows in ever
increasing health settings reflect key health
system needs that SNOMED International
must take into account as it evolves. SNOMED
CT, as a product, has a role to play in care
ecosystems that need information to flow
effectively, efficiently, and accurately; A task
which SNOMED CT has already helped our
Members to achieve seeing significant benefits
across varied clinical settings.
POPULATION HEALTH DATA
Increasingly structured
using SNOMED CT, data sets
can be explored to benefit
local, regional and national
interventions. Data collection,
extraction and analysis in the
recording and reporting of
smoking status and cessation is
one such example.
There are global patient care
outcomes and subsequent cost
savings to be realized where
Chronic Obstructive Pulmonary
Disorder (COPD) prevalence and
impact through hospitalizations
are concerned. SNOMED CT
enabled electronic health records
can code pre-determined COPD
terms and extract data to inform
future care planning decisions,
avoiding conventional data
collection and manual audit
processes.
