RIDDOR
OBSERVATION CARD
The Reporting of Injuries, Diseases and Dangerous Occurrences Regulations (RIDDOR) make the
reporting of certain accidents, ill health and dangerous occurrences to the Health and Safety Executive (HSE)
a legal requirement under the Health and Safety at Work Act
The purpose of the RIDDOR regulations is to allow the Health and Safety Executive (HSE) and Local
Authorities to monitor accident trends, identify how risks arise and investigate serious accidents.
Therefore all of us have a legal duty to report anything we see in the workplace that is not right and to cooperate with the Company to enable compliance with legal requirements
We need to be open and honest in learning from our mistakes, never being afraid to ask – talking about our
mistakes, changing the way we work, being better than we are, so the same mistakes do NOT happen
again. The question YOU need to ask yourself is…
Are we doing everything right
?
Talk to us we are keen to listen and encourage YOU to contribute YOUR thoughts and ideas.
We have several methods for reporting accidents, incidents, hazards, near misses, close calls or any other
observations. We want you to Speak up for Safety. It’s your chance to change things through
communication and involvement… doing things differently and better
The Occupational Health & Safety Management Standard, BS OHSAS 18001, provides the following definitions;
An accident is an incident which has given rise to injury, ill health or fatality
An incident where no injury, ill heath, or fatality occurs may also be referred to as a “near-miss”, “nearhit”, “close call” or “dangerous occurrence”
Safe Systems of Work need to be safe
Speak up if you believe improvements can be made
