Allied Health Professional (AHP) Preceptorship
Standards and Framework
Empowering new beginnings - building con昀椀dence for AHPs transitioning into new roles or workplaces.
As part of the ambition to ensure
sustainable future staf昀椀ng and
improve patient care, the NHS
England Long Term Workforce
Plan identi昀椀es good quality
preceptorship for the wider
workforce as key to supporting
recruitment, supporting staff
to integrate into their places
of work, and supporting staff
retention.
Allied Health Professionals (AHPs)
have been providing support
to newly registered AHPs and
those transitioning into the
workforce or new roles for many
years, but from our work with
services across the country, we
know there is wide variation in
the content and delivery of that
support offer.
Ensuring AHPs have inclusive
access
to
evidence-based,
high-quality
preceptorship
across a range of transitions
into employment, will help
them grow in con昀椀dence as
autonomous and accountable
practitioners,
enhance
their
career development, improve
recruitment
and
retention,
and ultimately improve patient
care. This also supports the
development of a diverse AHP
workforce 昀椀t for the future by
embracing the principles and
focus of the AHP Strategy for
England:2022-2027 AHPs Deliver,
in ensuring that there are AHPs
in the right place, at the right
time, with the right skills whilst
acknowledging the need to bridge
the gap between education and
work by optimising the con昀椀dence
and capability of those new to
role.
We have worked closely with a
wide range of partners to develop
the AHP Preceptorship Standards
and Framework, which should
be used in conjunction with the
Health and Care Professions
Council
(HCPC)
Principles
for Preceptorship, to provide
best practice guidance on the
application of these principles for
AHPs.
In line with the NHS England
Long Term Workforce Plan, the
AHP Preceptorship Standards
and Framework will play a
fundamental role in ensuring we
retain, upskill, and maximise the
potential of AHPs for the future.
Surgical swabs are sterile pieces of gauze which are
used to absorb bodily 昀氀uids, such as blood, during a
surgical procedure.
The investigation will:
• explore the factors associated with unintentional
retained surgical swab events
• identify alternative safety controls to reduce the
likelihood of foreign objects being unintentionally
retained.
The interim report analyses the 昀椀ndings of 31 NHS
trust serious incident reports.
Reference event
The reference event involves a patient who had
undergone a triple coronary artery bypass graft
surgery (heart surgery).
Following their surgery, a chest X-ray identi昀椀ed
that a surgical swab had been retained. The patient
returned to theatre and the surgical swab was
removed. A subsequent chest X-ray identi昀椀ed that a
further surgical swab remained in situ, in the same
location within the chest. The patient returned to
theatre and the second surgical swab was removed.

Investigation summary
This investigation explores the patient safety risk of
unintended retention of surgical swabs after surgery.

Safety Recommendations
NHS England
HSSIB recommends that NHS England incorporates
the 昀椀ndings of this interim report into its review
of the Never Events policy, with speci昀椀c focus on
considering removing retained surgical swabs as a
sub-set of retained foreign object Never Events.
Safety observation:
Organisations can improve patient safety by
using consistent terminology in national and local
guidance when describing the responsibility for the
reconciliation of items used in surgery and invasive
areas, including swabs.
Local-level observation:
Healthcare providers can improve patient safety by
using the 昀椀ndings of this report to consider potential
challenges in their own systems and processes
for unintentionally retained swabs following
invasive procedures. This can help organisations to
understand what people focused and system focused
barriers may be implemented to help further
mitigate against retained swab events.
