South Carolina’s Projects for Assistance in
Transition from Homelessness (PATH) program is
funded by the Substance Abuse and Mental Health
Services Administration (SAMHSA) and
administered by the South Carolina Department of
Mental Health (SCDMH). PATH funds are used to
provide outreach and clinical services to those
experiencing homelessness who have mental
illnesses or co-occurring mental illnesses and
substance use disorders.

STATE OF HOMELESSNESS REPORT
PATH provider agencies provide outreach and
clinical services to persons with mental
illnesses who are experiencing homelessness.
There are four PATH provider agencies in
South Carolina, one in each of the four HUD
Continuums of Care.
Persons with mental illnesses who
