Listening and responding to
community needs
“Understanding the key health needs of the community we serve is always a priority,” says
Caitlin Tilley, Director of Care Coordination. Toward that end, SVMC conducts a comprehensive
tri-annual Community Health Needs Assessment (CHNA). Using data gathered through
community forums and online surveys, the organization determines the priority
health needs and invests in community health projects, organizations, and
initiatives aimed at addressing the revealed issues.
“This past year, we made a concerted effort to include underrepresented segments of
the community to reveal what disparities exists,” says Tilley. “In the past we relied
on community partners to share this insight, but we knew we could do better. We
promoted response to the survey through posters with QR codes at COVID vaccine
sites, including the COVID Resource Center, and provided access to technology, if
needed, so individuals could complete the survey while waiting after receiving their vaccination.”
This inclusive approach will inform and guide the work done, both within and outside of the
hospital walls, to address emerging and ongoing health issues.

Community concerns revealed
In early 2021, SVHC hosted a series of CHNA online community
forums in each of the five regions it serves. Participants
shared healthcare experiences and discussed ideas for
improving the health of their specific community. In addition,
a digital survey promoted extensively in local media, on
social media platforms, and on the health system’s website,
captured feedback from nearly 1,650 individuals.
When asked ”what are the top three health needs that you feel should be prioritized
and addressed in your community?,“ survey respondents shared the following concerns:
Community cohesion, gathering
Mental health
Environmental health
Substance abuse
Primary prevention, healthier behavior
Affordable housing, homelessness
Domestic violence
Transportation
LGBTQ health
Access to care
Food security
