Improving outcomes through
inspired innovation
When SVMC’s leadership noted an increased rate of chronic
illness associated with advanced aging, they launched a
program to keep older adults out of the hospital. The program,
called Transitional Care, has continued under the leadership
of Chief Nursing Officer and Vice President for Patient Care
Services Pamela Duchene, PhD, APRN. By pairing transitional
care nurses with primary care providers, the program identifies
problems, reduces readmissions, addresses medication
complexities, and improves patient satisfaction. The innovative
thinking and responsiveness by leadership captured the
attention of the American Hospital Association (AHA),
and in February 2021, SVHC was honored to receive
AHA’s Rural Hospital Leadership Award.
Cancer Center receives accreditation
Renal Dialysis earns high marks
The Southwestern Vermont Regional Cancer Center’s commitment to excellent care
and support was recognized by the American College of Surgeons which granted it
a three-year accreditation. Requiring a multidisciplinary approach to treating the
complexity of cancer, the accreditation validates that SVRCC patients are receiving the
very best care possible.
SVMC’s Renal Dialysis Unit earned a prestigious five-star
rating based on clinical quality and patient experience from
the Centers for Medicare and Medicaid Services. In addition
to this accolade, the department underwent a rigorous
three-day survey and licensure review
and was determined to be
deficiency-free.
